11/7/2023 EnchantaVaughn Approved Behavior: QMHPA arrived to the home of Kaila. Upon arrival to the home, Kaila was not dressed or prepared for the session. Kaila required prompting to focus for the session. Kaila communicated her mood and how she was feeling as the session started. Intervention: QMHPA began the session by assessing Kaila current emotional well-being. QMHPA utilized active listening to provide Kaila with constructive feedback regarding her mood. QMHPA prompted Kaila for the session. QMHPA worked with Kaila on learning how to express her emotions appropriately. QMHPA educated Kaila on the importance of developing healthy interpersonal relationships. QMHPA emphasized the importance of Kaila using assertive communication styles when meeting new people; and to avoid aggressive styles. QMHPA supported Kaila in understanding how engaging in more social settings can increase her daily living and reduce isolation. QMHPA encouraged Kaila not to isolate herself from others Response: Kaila appeared to be receptive to counselor’s interventions and services that were provided to her during the session. Kaila required some prompting and redirection throughout the remainder of the session. Kaila was able to be prompted and participated in the session. Kaila learned how to express her emotions in an assertive manner and not be aggressive towards others. Kaila required some assistance with processing the importance of healthy interpersonal relationships. Kaila noted that she did not like people and like being by herself. Kaila was encouraged to seek age-appropriate social settings that could increase her support system. Kaila was able to recall how to express her thoughts and feelings assertively. Kaila practiced utilizing assertive skills such as looking one in the eye, being mindful of ones’ tone and posture. Kaila was encouraged to practice these skills independently. Kaila notes her struggles with controlling her emotions when communicating with her family and peers. Kaila is reminded to work on building a healthy relationship with her family and to avoid conflict in the home. Plan: QMHPA will utilize role play techniques addressing appropriate and inappropriate behaviors in social settings (1-2x weekly).
